HSP Intake Forms
What you should bring to your first appointment:
• Insurance card or claim information
• Intake Forms filled out and printed from home
• Referral from your doctor
• HSP Adult Admission Form
• HSP Pediatric Admission Form
• HSP Social Language Form
• HSP Disclosure Record Release Form
Both the adult and pediatric intake packet include a "release of information" page. We do not require you to fill this out, but suggest that you complete the release page if you'd like us to obtain information from or communicate with another provider you or your child has seen.
Click the links above to view and/or print the intake paperwork for your first appointment. You may bring paper copies to your appointment, fax or mail the completed paperwork, or scan and e-mail a completed copy to our office.
HSP accepts most major insurance plans. As a courtesy, we can call your insurance company to verify coverage for therapy in our office.
Benefits quoted by an insurance representative are not a guarantee of payment and at times, can be quoted incorrectly.
We encourage you to call your insurance company prior to your first appointment to verify that you are covered for therapy in our office. We also accept Auto and L & I claims from insurance companies. Private Pay rates are available upon request.
